Lifetime Dental is not a licensed insurer, health maintenance organization, or other underwriter of health care services. The Plan is a direct agreement between plan members and Lifetime Dental only. The Plan benefits do not apply to any referred outside specialist. A new family member may be added to Smile Care at any time. All family members must live in the same household. The annual Membership Fee is not refundable, and Membership may not be rolled over, transferred, or sold to other family members or individuals. Payment in full is required for any treatment at the time of service to be eligible for Membership Discounts. Dental services, not products, are eligible for Membership Discounts.
